Good morning all. I applied to have C1 put back on my license due to DVLA taking it off because I have insulin. I used D4Drivers and had an appointment with a consultant who checked everything and carried out eye test, blood pressure etc etc etc he detailed every medication and listed my meds and various consultants. There literally was nothing left to answer so ( along with a 2 day old ( original ) full report from my own diabetes consultant ) I sent the D2 and D4 forms off with my license.
I’ve now, after 4 weeks, received a VOCH1 form from DVLA to fill in ? It’s pretty much everything what was asked on the D4 form which I paid for !!
Has anybody come across this before ? I don’t understand why they’re asking me everything which is on the forms they received from me ?
I’m going to “try” and phone them on Monday but just thought I’d ask on here incase anybody had the same ?

Thanks for the reply, yeah it was DVLA who asked me to send them a D2 and D4 form in, which I’ve done. They’ve now sent this VOCH1 form , which is the same questions ?
 
Did you manage to get hold of them today @Bobbingbuoy ?

Are things any clearer?
